Enrollment and Waiver Information

Welcome to the UnitedHealthcare Student Resources Online Student Center. Please read the information below and select the appropriate link to either the Waiver Form or the Enrollment Form.

All USM students will be required to show proof of having health insurance as a condition of course enrollment with each new school year if they meet the following eligibility criteria:
  • Undergraduate students taking 9 credits or more
  • Graduate or Law students taking 6 credits or more
  • All USM School of Nursing students enrolled in clinicals
  • All USM Athletic Training Program students enrolled in clinicals
  • All USM students enrolled in the Occupational Therapy Program

Note: You will need your student ID number to log-in to complete either an enrollment or waiver.


SUBMITTING A WAIVER REQUEST:
  • Prior to completing an insurance waiver form, we strongly recommend that you review the Student Health Insurance Policy (SHIP) plan options available at: http://www2.crossagency.com/2015-2016/usm.php
  • You will need specific information about your current insurance policy to complete the on-line waiver form.
  • Your waiver request must be completed in its entirety. Failure to do so may result in a non-refundable billing for the student insurance plan
  • You may not make changes or re-enter the waiver once it is submitted.
  • Immediately after completion of the waiver, you will receive a confirmation of approval or denial via email. Please print this confirmation for your records. If you don't receive a confirmation, your waiver attempt was unsuccessful. Please contact Health and Counseling Services at 207-780-5411 for assistance.
  • You must submit a waiver request online upon entry to USM as well as each subsequent fall semester during your tenure at USM. If you are beginning your studies at USM during the spring semester, you will be required to complete another waiver prior to registering for the following fall semester.
  • All information provided is retained in a secured environment and will not be provided to unauthorized individuals
  • Insurance must be continuous throughout your tenure at USM. If, after submitting your waiver request, your insurance plan is discontinued, you are required to report this change to USM Health & Counseling Services at 207-780-5411.
  • Insurance waiver information is subject to audit both by USM and/or by United Healthcare Student Resources.


SUBMITTING AN ENROLLMENT CONFIRMATION:
  • To confirm your enrollment in the Student Health Insurance Policy (SHIP), please complete the online enrollment form by clicking the link below.
  • After completing the enrollment form be sure to "Create an Account" to have 24hr access to your policy and ID Card
  • Please be advised that your coverage will not begin until the effective date of the policy (01/1/15). An ID card will be mailed to the address on file two weeks prior to that date.
