All registered students enrolled in one (1) or more credit hours are required to participate in the Student Health Insurance Plan and premium is automatically billed on the tuition billing statement unless proof of comparable coverage is provided by completing the waiver link below.

Waiver requests must be processed by:
September 30, 2015
No waiver request will be accepted after the posted deadline. Your account will be charged for the student insurance unless your waiver request has been approved.
Submitting a waiver request:
You will need your Pacific University student ID number and an email address that you can be reached (preferably your Pacific University email account) to verify your entry to the waiver card site.
Your waiver request must be completed in its entirety. You will be asked detailed information about your current coverage, so have the following available:
Insurance company name, policy/group number, insurance company contact information.
Policy holder’s name, address, telephone number, and date of birth.
You may not make changes or re-enter the waiver once it is submitted.
Immediately after completion of the waiver, you will receive a confirmation of approval or denial via email. Please retain this email confirmation for your records.
The waiver period for your school has not started or it has already ended. Please contact your school for more information.
